Mobs Randomly Disappearing Then Reappearing

at times when fighting world mobs, they will disappear mid fight and then reappear several seconds later. they are still engaged in battle and often show up again near the same place they disappeared. so far, this hasn't been exclusive to one area or one type of mob.
