Golden Eye Mantutu encounter not spawning levers

This encounter has a mechanic where the boss gets buffs that you remove using levers that spawn during the encounter. Those levers are not spawning at all, making the encounter pretty much impossible even for our group of well geared 45-50's.

This also prevents kill credit for quest [Coin] Safecracking.
